The privacy and security of your personal medical record is addressed in State and Federal laws and regulations, most notably the Health Insurance Portability and Accountability Act (HIPAA). Rapid City Regional Hospital and its affiliated hospitals and clinics are dedicated to protecting the privacy of our patients, and to fully implementing the Privacy and Information Security requirements as stated in the HIPAA regulations.

Notice of Privacy Practices (NPP)

To exercise your rights as stated in the Notice of Privacy Practices, you must follow the guidelines listed in the Notice. Links to required forms are listed below. By clicking on the link, you can download a copy of the form. The completed form must be returned as directed in the notice of privacy practices.

Authorization for Disclosure/Release of Medical Information (PDF)
This form is used when you want us to release copies of your records to a third party. It is also used when you request to view or receive copies of your record. It must be completed and returned to the Medical Records Department at the facility that maintains your records.

Request for Correction of Your Medical Record (PDF)
If you have reviewed your record, and believe that there may be errors in the record, you may request a correction using this form.

Request for Report of Disclosures (PDF)
From time to time, we may required to report portions of your record to outside agencies. If you want to request a report of disclosures that were not for Treatment, Payment or Healthcare Operations, you may use this form.
