Thank you for taking a step today to benefit your health care by filling out and printing a medication card. In doing so, you are partnering with Regional Health to improve the care and safety of you and your loved ones.

Please click on the link below to fill out and print the card entitled "My Medication Card." Please carry your card with you at all times and update it whenever there are changes. Paper versions of the cards are also available at Regional Hospitals, Regional Medical Clinics, and Regional Senior Care facilities.
